Treatment Of BPH With Transurethral Resection

For men after forty years, the disease of prostate adenoma is significant. This disease occurs when the tissue of the gland is enlarged. This process in itself does not cause pain to the patient, but its consequences are unpleasant.

When should a resection be prescribed

The prostate gland is located in the area of the urethra and during prostate adenoma disease, the canal is under pressure. This circumstance leads to squeezing of the canal and stagnation of urine, as a result of which cystitis, pain in the lower abdomen, burning sensation occurs. At a later stage in the development of the disease, sand and stones develop in the bladder.

Adenoma of the prostate is now called BPH (benign prostatic hyperplasia). This disease is a benign tumor that does not give complications and, regardless of the manifestation of painful symptoms, is not a dangerous cancer. But, nevertheless, it is imperative to treat the disease.

And one of the options for treating the disease is TUR of the prostate. TUR operation is assigned in case of:

  • Frequent urge to use the toilet;
  • Constant sensation of urine in the bladder;
  • Efforts must be made to empty completely;
  • The occurrence of painful sensations, cuts and burning during urination;
  • Appearance of blood in the urine;
  • The bladder empties slowly and intermittently;
  • The bladder is often affected by infections;
  • Fear of loss of sexual function at this stage of life;
  • The appearance of stone formations in the prostate or bladder;
  • Appearance of kidney disease;
  • After surgery in the lower abdomen;
  • Doctors are prohibited from open surgery.

TURP of the prostate is a surgical intervention that is gentle and causes a small number of injuries, due to which the removal of the neoplasm occurs with minimal complications in the postoperative period.

Benefits of a TOUR operation

TURP of the prostate gland differs from open surgery by a large number of positive factors.

Of these advantages, let's highlight the main ones:

  • TURP of the prostate can be done to people of a fairly young age. This operation will not affect his sexual function.
  • Thanks to this surgical intervention, oncology is diagnosed on time.
  • Resection is performed even if the man is overweight.
  • In case of diseases of the urinary system, TUR surgery on the prostate is not prohibited.
  • In case of recurrence of prostatitis, surgery with TUR provides a complete diagnosis and treatment of the patient.
  • TUR is the most effective treatment for prostatitis.

Contraindications to surgery

Regardless of the fact that in the treatment of the prostate, the operation is completely harmless, there are still contraindications to it, which are expressed in the following aspects:

How the operation works

The operation takes an hour and a half. For anesthesia during surgery, epidural anesthesia is used, that is, an injection is made in the spine. During the surgical intervention, the patient is aware of where he is and what is happening to him, but does not feel his body below the lumbar spine at all. If the patient has a fear of surgery, and he wants to completely not see the course of the procedure, then he is given general anesthesia.

During preparation for the operation, the patient is placed on his back, legs are spread apart and the genitals are thoroughly treated with antiseptic drugs. The urinary canal is treated with a special gel and then proceeds directly to the operation itself.

This is a kind of loop with the help of which the affected tissue is removed under the influence of an electric current.

As a result of the operation, damage to the blood vessels occurs, which significantly interferes with the doctor's ability to conduct the operation

The tissue that is cut off during the operation is sucked off with a special pump. At the end of the surgery, all vessels are completely checked for their integrity and the instrument is removed.

After that, I insert the catheter, and with the help of a balloon, fluid is pumped in to plug the damaged area. This is done in order to stop internal bleeding that occurs due to damage to small vessels. Thanks to this, the urinary canal will not become clogged with blood clots.

In this case, the bleeding lasts for four days. For this reason, the catheter is not removed from the urethra.

Complications after surgery

After surgery, patients may experience unforeseen complications, which include:

Nosocomial infection.Even if the operation takes place under sterile conditions, the patient is in the hospital in the postoperative period. In these conditions, there is a chance of contracting infectious diseases. At the same time, during the operation, there is still a 30% risk of getting infected during the operation itself. In this case, the microorganisms have immunity to drugs. Therefore, the treatment will be difficult and lengthy, during which antibacterial agents are used.

Retrograde ejaculation. In this case, semen enters the bladder. A similar complication occurs in 70% of cases. This complication does not lead to serious illness, but at the same time there is a problem with conception. This problem lowers a person's self-esteem. The complication is treated in two ways: surgical intervention and conservative method.

Changes in the structure of the urethra. After surgery, there is a risk that the urethra will decrease significantly. Then a new problem will appear. In the process of constriction, stagnation of urine in the bladder will occur. This will provoke an infection of the bladder, its enlargement and kidney damage. Urology for the treatment of this pathology uses bougienage, that is, the urethra is expanded with the help of probes. In addition, specialists can prescribe a surgical intervention, with the help of which plastic of the urethra is done.

Impotence. This complication in the postoperative period occurs extremely rarely. But nevertheless, there is one percent of the occurrence of this disease. in this case, conservative treatment is carried out, during which drugs are prescribed that can improve blood circulation in the genital organ. Or this problem is solved with the help of surgical intervention.

Urinary incontinence. You can cope with this complication with the help of exercises aimed at strengthening the muscles of the pelvic floor. In more difficult cases, a bladder sphincter implant is placed.

Recovery period

In the postoperative period, first of all, they closely monitor urine tests. In the event that no traces of blood are found in it, then this is a good sign of a successful operation and a reason for being discharged from the hospital. During the recovery period, the patient does not feel any pain symptoms, apart from discomfort from the catheter.

There are cases when the patient feels spasms, but they are eliminated with the help of drugs. It may be difficult to go to the toilet. In this case, experts give recommendations for certain actions, with the help of which the process is greatly simplified. Alternatively, the catheter is reinserted. After this procedure, the problem disappears completely.

Important to know! Since in the postoperative period the bladder remains sensitive to any irritants, you should follow a diet for some time. On the first day, you are only allowed to drink water in small quantities. Do not eat salty, smoked, fatty and fried foods.

After discharge, at home, the patient must adhere to the following rules throughout the week:

  • Complete lack of sexual activity.
  • Move less.
  • Avoid constipation.
  • Drink more water, teas to stimulate the bladder.
  • Do not consume alcoholic beverages.
  • Refrain from traveling in vehicles.

In 80% of cases, surgery is successful. In this case, the patient feels some discomfort, which disappears as a result of the correct treatment.

In case of removal of a malignant neoplasm, the patient is prescribed additional drugs, with the help of which cancer cells are completely removed. For this, chemotherapy and radiotherapy are used. In this case, the rehabilitation period lasts longer than usual.
